Glove Box Accident and Information Form - Keep Handy in Case of an Auto Accident

(Keep this in your glove box and fill out if you get in a car wreck)

What to do after an auto accident:

1) Call an ambulance, or go to your family doctor immediately after the
accident. It is very common that you might not “feel” hurt at the scene due to shock. You will probably begin
to get stiff and sore in the hours and days following the accident. At least go to the ER or your doctor to get checked
out. Call us if you need assistance in finding a doctor.
2) Call the police, tell them there were injuries, and insist they
come out and write a report. The police report may be vital to helping you prove your case
later. The police will not come out unless you tell them someone was injured.
3) Take pictures of the scene, vehicles, and other driver. (Carry a
disposable camera in your glove box for this purpose.)
4) Except to get the information below, do not talk to the other
driver, and make no admissions or apologies.
5) Get names and telephone numbers of any witnesses.
6) Call Street & Ragsdale as soon as you are able.
7) DO NOT GIVE A STATEMENT TO THE INSURANCE
COMPANY! Let us do the talking for you. It is NEVER to your advantage to give a statement to an
insurance company.
